This is only in one of the smaller (4x4) rooms, with one of the Gentlefolk spirits. Occasionally, one of them will develop the "lost" condition (? icon above their head) and then temporarily "jump" out of the room (it almost immediately resets the spirit). AFAIK it's at full peace etc.
